My OPA695 amplifier's voltage

Good afternoon~thanks for taking time to read my question,I hope I came the right froum. Well,time is money so let's enter into the subject directly.My trouble is about "The voltage about OPA695 amplifier".(this is OPA695 I used)Let me describe my problems clearly while you can review my circuit.

Last days,I have printed a board after I made a circuit by using simulation software. However, The voltage which have tagged in the picture is different from the simulate value when I texted the board. In fact,the voltage of R9 two sides in the board just show 0.5V, and R8 shows 4.6v. I can’t understand why occurs such a question? I have review carefully the PCB board,there aren’t any wrong. Can you help me solve it?

Make sure the pinout of the opamp is correct in your layout and the opamp is inserted in teh correct direction.
The datasheet shows the top view.

R8 and R9 are connected together on the schematic but since you measure different voltages on them then they are not connected together in your circuit.
