Relay With Circuit Board Feedback

Hello,
I am looking for some guidance for a contact closure relay with feedback. I need to add a relay to something in my home that I can connect into my home automation system. I need it to be able to communicate back to my system saying it is either open or closed. I have not seen a relay that can do this operation, so is there a addon circuitry that I can add that gives me this feature.

Thank you for the reply. I do understand DPST or DPDT, but so I understand how would that feedback to my controller showing in what state of the relay.

How did you want to communicate?
Bluetooth? Hardwired? serial?

I would prefer hardwire, RJ45. But at this point, not to picky.

Pretty standard interposing relay arrangement.
Drive the coil from whatever in the system to be monitored, then the dry /voltage free contacts feed back the closure of the relay to the monitoring equipment.
Could also be any number of other alternatives such as opto isolator.
